The Clinic will ask for billing and descriptive information, including:
1) Name, address, phone, date of birth
2) Description of the problem
3) Pertinent medical, school or other background information

Prior to the appointment, we will mail materials to the patient’s family, including a history form (for children), appointment confirmation, and map. Waiting times for appointments may range from one week to one month. Some early evening or weekend appointments are available.
