The Clinic will ask for billing and descriptive information, including:
1) Name, address, phone, date of birth, social security number,
2) Description of the problem
3) Pertinent medical, school or other background information.

Prior to the appointment, we will mail materials to the patient’s family, including a history form (for children), appointment confirmation, and map. Waiting times for appointments may range from one week to one month. Some early evening or weekend appointments are available.
